Adult Protective Services Case Manager
Jefferson County’s Aging and Aging & Disability Resource Center (ADRC) Division provides information and assistance to a diverse community where the elderly and people with disabilities are respected, healthy, and productive. It is their mission to help people achieve their goals by providing them with comprehensive information so they can make informed decisions, and advocate support to ensure that they remain in charge of their lives.
The Aging and ADRC Division is hiring an Adult Protective Services (APS) Case Manager to join their agency and provide services to elderly and adults with disabilities. Services include receiving and responding to reports of abuse and/or neglect, providing and monitoring emergency and non-emergency protective placement and guardianship services, and providing case management services to individuals through the Targeted Care Management (TCM) and Emergency Mental Health programs.
As a knowledgeable, experienced, and helpful APS Case Manager, you will be successful in this role by completing the following key tasks and responsibilities:
Receive, respond, and investigate reports of abuse (physical, sexual, financial, or emotional) and neglect (self and/or from others) of vulnerable adults and elderly. Determine if emergency vs non-emergency and react appropriately
Receive requests for county funded guardianships and protective placements. Prepare court paperwork properly, including Comprehensive Evaluation. Monitor and prepare the Annual Reviews of Protective Placement and attend court hearings as necessary, including testifying when needed.
Provide ongoing Crisis Intervention services who do not have a primary diagnosis of mental illness and/or who are served via the Comprehensive Community Services (CCS), Community Support Program (CSP), or the mental health teams.
Monitor Chapter 55 protective placement and services court orders.

Starting wage: $24.18/hour; 40 hours per week

Bachelor’s degree in Social Work, Human Services, or related field; or equivalent combination of education and experience
Valid driver’s license
One-year experience working with one or more of the client populations served by the ADRC
Master’s degree in Social Work or related field with two years of experience working with one or more of the client populations served by the ADRC
Licensed Social Worker through the State of Wisconsin
