The Community Support Program (CSP) Resources toolkit is available thanks to the combined efforts of numerous CSP professionals, DHS staff and the Wisconsin County Human Service Association (WCHSA). A variety of forms, policies and other tools are contained in this resource kit and have been supplied from a wide range of CSP programs throughout the state. These resources did not originate from DHS or WCHSA and should not be considered vetted as “official,” or mandatory, nor even best-practice. The contents are intended for use by any CSP program throughout the state and can be modified to meet the needs of individual programs. These resources will be reviewed on a regular basis to ensure they continue to be relevant and useful.
